I accept Horizon Blue Cross Blue Shield and Medicare.  If you have a Horizon or Medicare insurance plan, you will be responsible at the time of session for your co-pay, which should be listed on your card.  

 

For other insurance networks, I am an out of network provider.  You may choose to file for out-of-network reimbursement with your insurance provider.  If you choose this option, I will provide you with a detailed receipt that you can submit directly. Typically you will have a deductible you'll need to meet prior to any reimbursement for out-of-network services being provided.  Please contact your insurance provider for your benefit details.

You may prefer not to file for reimbursement with your insurance provider.  This allows you to have more choice in who you see and for how long without having to worry about prior authorization or insurance approval.  You are also ensured full privacy since I am not obligated to provide your insurance company with confidential information regarding your mental health, diagnosis, or other protected health information.

Please note that Gestalt coaching for writers is not eligible for insurance reimbursement.

 

My standard fee is $180 per 55 minute session.
